Topical antifungal medications used in the treatment of chronic rhinosinusitis (CRS) include amphotericin B, itraconazole, and voriconazole. Currently, topical preparations of these antifungals are not commercially available. Therefore these antifungals must be compounded or made extemporaneously. Although antifungals can and have been compounded by many pharmacies, it is essential that the prescriber ensure that proper compounding procedures and techniques are in place to ensure the safety and efficacy of compounded antifungal products used in the treatment of CRS. In addition, compounding pharmacies must ensure compliance with state and federal regulations, such as those defined in USP Chapter (797), Pharmaceutical Compounding-Sterile Preparations.
To ensure efficacy, preparation by a certified compounding center is recommended. Certified centers have been reviewed and are in compliance with regulatory guidelines and protocols for preparing the topical antifungal medications appropriately by using the appropriate drug delivery vehicle and procedure. They can also provide patient education materials and support.
